Understanding the Treatment

What is Arthrocentesis for TMJ Pain?

Arthrocentesis is the simplest surgical procedure for the jaw joint. Two fine needles are inserted into the TMJ space under local anaesthesia, and sterile saline solution is flushed through the joint. This washes out inflammatory chemicals, breaks down any adhesions (scar tissue bands) that are causing the jaw to lock, and stretches the joint to restore normal movement. It is a day procedure requiring no general anaesthesia and most patients return home within an hour.

Rest for the remainder of the day. Mild soreness and swelling over the joint is normal for 2-3 days. Apply ice packs to the jaw for the first 24 hours. Eat soft foods for 3-5 days. Begin the gentle jaw exercises we prescribe to maintain the range of movement achieved during the procedure.

Frequently Asked Questions

Answers from Dr. Deepthi Kalahasti and the GardenCity Smiles team.

The procedure is done under local anaesthesia so you feel no pain. Post-procedure soreness over the joint for 2-3 days is managed with anti-inflammatory medication.

Studies show 70-80% of patients experience significant improvement in mouth opening and pain reduction after arthrocentesis. Results are best when followed by splint therapy and jaw exercises.

Arthrocentesis uses needles to irrigate the joint — no camera is involved and it is simpler. Arthroscopy uses a tiny camera to visualise and treat the joint but requires more equipment and expertise. Arthrocentesis is the appropriate first surgical option for most closed lock cases.

Most patients need only one procedure. If symptoms return, a second arthrocentesis or progression to arthroscopy may be recommended. Long-term splint therapy significantly reduces the chance of recurrence.
