Gum disease is a bacterial infection of the structures supporting the teeth — the gums, periodontal ligament, and jawbone.
Stage 1 — Gingivitis (reversible): Plaque accumulates along the gumline. If not removed, bacteria release toxins that inflame the gum tissue. Gums become red, puffy, and bleed easily. At this stage, the bone and ligament are not yet affected, and with professional cleaning the gums return to complete health.
Stage 2 — Periodontitis (not fully reversible): If gingivitis is untreated, infection spreads below the gumline. Bacteria colonise deep pockets and the body’s immune response destroys surrounding bone. Teeth become loose. Once bone is lost, it cannot be fully regrown — only managed and stabilised.
The systemic link: Gum disease bacteria enter the bloodstream and have been strongly associated with heart disease, diabetes, preterm birth, respiratory disease, and Alzheimer’s disease. Treating gum disease improves blood sugar control in diabetics.

For gingivitis, a professional cleaning to remove plaque and tartar above and just below the gumline, combined with oral hygiene instruction, is all that is needed. Gums typically return to health within 2 weeks. Recommended every 6 months for prevention.
Scaling & Root Planing (Deep Cleaning)
For periodontitis, an ultrasonic scaler and hand instruments remove bacterial deposits from deep within gum pockets and smooth the root surface under local anaesthesia. Smooth roots are harder for bacteria to re-colonise. Done in 2–4 sessions.
Laser-Assisted Periodontal Therapy
At GardenCity Smiles, we use dental laser to selectively remove diseased tissue inside periodontal pockets with minimal bleeding, faster healing, and less post-operative discomfort. The laser also decontaminates the pocket with its bactericidal effect.
Locally-Delivered Antibiotics
After deep cleaning, slow-release antibiotic gels can be placed directly into deep pockets to eliminate remaining bacteria. Reduces the need for surgery in many cases.
Periodontal Surgery (Flap Surgery)
For deep pockets that do not respond to non-surgical treatment, gum tissue is gently reflected to allow direct cleaning of root surfaces and reshaping of bone defects. Bone grafting can be performed simultaneously.
Supportive Periodontal Therapy (Maintenance)
Once treated, periodontal disease requires ongoing 3–4 monthly professional cleaning to prevent recurrence. Periodontitis is a chronic disease that is managed, not cured.

One of the most dangerous things about gum disease is that it is largely painless — especially in the early and middle stages. Many patients discover they have significant bone loss only when teeth start to feel loose. By then, the damage cannot be undone.
My gums bleed so I do not brush there — this is the most common and most harmful patient behaviour. Avoiding an area because it bleeds allows plaque to accumulate further, worsening the inflammation. Healthy gums do not bleed. Bleeding is a sign of infection, not a reason to stop cleaning.
